Care Management

Service Description

Our care management program is designed to help people stay in their communities and reduce the risk of institutionalization. We work closely with our clients and their supporters to stabilize their quality of life. This includes, but is not limited to, helping them navigate multiple systems of care, accessing needed services, and coordinating appointments and service delivery. Additionally, the program offers care management consultation and advocacy for clients living in long-term care placements.
